• Debts and Liabilities - Name /address of the creditor, monthly payment and balance due for each outstanding loan; Credit card bills for past few billing periods; Other consumer debt like car loans, furniture loans, student loans and other personal and cosigned installment loans with creditor addresses and phone numbers; Evidence of mortgage or rental payments; Copies of alimony or child support
• Current and Previous Addresses - Property address, current market value, current mortgage lenders name, account number, current monthly payment, and outstanding balance if you won your own home. Current address with name and address of your landlord or management company, and monthly rent amount if you rent.  
• Information about the Purchase - Copy of the ratified purchase contract. If you made a deposit to the seller to show that you are serious about buying the house, bring a copy of canceled deposit check on house.
• Separated or Divorced? - Copy of your divorce decree and separation agreement and documentation about any alimony or child support payments you make. If you receive alimony or child support and would like the lender to consider these monies as income for the purposes of their calculations, provide proof of these payments for the past 12 months.
• Bankruptcy, Foreclosure or any Judgements Against You in the Past 7 Years -  Documentation for a bankrupcty should include copy of the bankruptcy discharge and a schedule of both debts and assets.  Judgments against you should include attorney’s letter that explains the outcome of the proceedings.
• Federal Housing Administration (FHA) Loan Application - Photo ID card, such as a drivers license. You may also need to bring your Social Security Card.
• Department of Veterans Affairs (VA) Loan Application - Your DD214 Form (discharge papers) or certificate of eligibility.

Penalties are usually expressed as a percent of the outstanding balance at time of prepayment, or a specified number of months of interest.
• Miscellaneous fees - Miscellaneous costs that may be incurred during the processing of your loan request, such as notary, courier, county recording fees and title company escrow fee,etc.
